Job summary
An exciting opportunity has arisen for a Advanced Clinical/Nurse Practitioner, to work with us in our modern newly built Medical Centre in Acklam, Middlesbrough. Acklam is on the outskirts of Middlesbrough, close to the Cleveland Hills and North Yorkshire Moors.
This role is for 5 sessions per week on the following shift pattern.
TUESDAY - 8am-6pm
WEDNESDAY 8am-6pm
EVERY OTHER FRIDAY - 8am-6pm
You would be joining our friendly, supportive, and rapidly expanding team. Excellent clinical support is provided from our extended clinical team, including GP's, ANPs, First Contact Practitioner, Social Prescribers and Prescribing Pharmacist.
Main duties of the job
To practice as a Advanced Clinical/Nurse practitioner without direct supervision but within the vicinity of a senior clinician offering support when required.
To assess , plan, treat and refer appropriately patients requiring the clinical service and evaluate patient care as appropriate
To recognize the need for, initiate and where appropriate interpret relevant diagnostic investigations.
To recognize the need for and initiate where appropriate to act upon the advice of relevant specialisms.
To exhibit excellent communication skills when dealing with patients. Non verbal and verbal skills required. To have the ability to make clear decisions with confidence and communicate these effectively.
To maintain appropriate clinical records, ensuring that the documentation clearly demonstrates the NMC Guidelines for Records and Record keeping.
To prescribe + review medication appropriate to patient needs in accordance with EBP + current guidelines.
To support patients to adopt health promotion strategies to promote healthy lifestyle.
To work within the parameters of his/her skills and knowledge, to recognise the limitations to ones own practice and to work within the bounds of ones training and capability according to the NMC Code of Conduct.
To maintain an awareness of developments in clinical practice.
To work effectively as part of a multi professional team in collaboration with other members of the service.
